The Albury Community Nursing Team is looking for suitably experienced Registered Nurses to join the dynamic team at Albury Community Health. The role of the Community Registered Nurse is that of coordination, delegation and leadership of nursing care within a community-based setting. It is expected that clinical care delivered by the Community Nurse will include complex care that requires specialist knowledge, skills and competence in areas of wound care, catheter care, CVAD care.
The Community Registered Nurse position provides community based post-acute, Community Home Support Program (CHSP) and Primary Health Care services to individuals and groups within the Albury and local community. This position involves assessment, planning, appropriate referral and accurate record keeping as well as the provision of direct clinical services to people with acute, chronic and complex care needs.
We are actively seeking candidates who meet the following mandatory requirements to apply for this exceptional opportunity.
Current Nursing/Midwifery Registration with APHRA (Australian Health Practitioners Agency)
Minimum of 3 years post graduate nursing experience in an acute care facility
Demonstrated understanding of the issues relating to the provision of home and centre-based community nursing and the principles of primary health care. Including knowledge of wound management, IV antibiotic administration, catheter care, care coordination and management.
Demonstrated ability to undertake a comprehensive client assessment and to work as part of a multidisciplinary team. Demonstrated ability to work unsupervised while maintaining accountability.
Demonstrated commitment to patient focused care and ability to plan and manage clinical activities.
Employment Type: This is a part time (up to 32 hours per week), fixed term position for a period of up to 12 months
