We are currently seeking a dedicated and compassionate Social Worker to join our esteemed community discharge to assess team. As a Social Worker in this role, you will play a pivotal part in conducting comprehensive Care Act assessments for individuals who have recently been discharged from the hospital into temporary 24-hour care placements. Your primary responsibility will be to assess their care needs, identify suitable services, and coordinate the implementation of appropriate support. The job role pays up to £30 an hour and is based in Bolton.

Responsibilities:
• Conduct thorough Care Act assessments for individuals who have been discharged from the hospital and require community support.
• Assess and evaluate the physical, mental, and emotional needs of individuals, as well as their functional abilities, to... determine the appropriate level of care and support necessary.
• Collaborate with individuals, their families, and healthcare professionals to gather pertinent information and develop comprehensive care plans tailored to their unique requirements.
• Provide expert advice, guidance, and emotional support to individuals and their families, empowering them to understand their available options and make informed decisions.
• Liaise effectively with relevant community resources, including healthcare providers and voluntary organizations, to ensure the seamless provision of necessary services.
• Advocate for the rights and entitlements of individuals, ensuring they receive the appropriate and timely support services they deserve.
• Maintain accurate and up-to-date records of assessments, interventions, and outcomes in strict adherence to organisational and legal requirements.
• Stay abreast of changes in legislation, policies, and best practices pertaining to social work, care assessments, and community support services.

Requirements:
• Degree in Social Work.
• SWE registered.
• Sound knowledge of the Care Act, Mental Capacity Act, and Best Interest Processes is crucial for this role.
• Familiarity with Continuing Health Care processes and participation in Decision Support Tools (DSTs) is desirable, though not mandatory.
• Enhanced, clean DBS. (We can provide this if needed.)
