Department Description
The PATH program (Projects for Assistance in Transition from Homelessness) is a program designed to provide outreach, engagement services and case management to individuals who have been identified by the state as having a serious mental illness who are homeless, at risk of homelessness or unstably housed. This position locates these individuals and then wraps services around them in hopes of getting them connected to outpatient behavioral health services and other services as needed. The Forensic PATH program’s overall goal is to divert individuals from the criminal justice system and instead help them get the services they need.
The Forensic PATH team will conduct outreach in partnership with existing PATH programs, local homeless continuum of care organizations to link individuals to housing services, treatment and resources. All effort shall be made to engage and build rapport with those individuals identified. Using a person-centered approach, Case Managers will assist clients to determine which services are needed such as securing funding, food, transportation, medical services, and other social services as needed. Connecting and referring to community behavioral health and substance use treatment centers is another key function.
Essential Functions
Provides outreach and screening to individuals who are in areas where homeless are known to congregate, including places not meant for human habitation and to specific areas identified by community partners.
Provides effective case management services to assure appropriate client-centered care.
Coordinates and maintains effective working relationships with contracting organizations and/or other community providers to include other Trueblood Settlement programs: Forensic HARPS, Outpatient Competency Restoration Programs and Forensic Navigators.
Assures that all administrative and clinical documentation, written and online, is completed in a thorough, accurate, timely and appropriate manner.
Completes all State required training and assists with tracking data elements and the completion of monthly and quarterly reports.
Secondary Functions
May provide back-up assistance to other Case Managers as assigned.
Attend all Program and Agency mandated trainings and meetings.
Job Scope
Works under general supervision of the AOS Manager. Manages a caseload of clients in a highly competent and professional manner. Work involves a significant variety of non-routine tasks of moderate complexity which are guided by formal policies and general procedures. While this position has limited direct budgetary accountability, errors in work can have significant impact on customer service, outcomes, and liability implications.
Communication / Customer Contacts
This position involves a high frequency of communication, in various forms, verbal and written, with clients, other staff, and community partners, including occasional community presentations. Must also abide by all confidentiality laws and pertinent codes of conduct.
Education and/or Experience
High school diploma/GED and 1 year of relevant experience. Associates degree preferred. Experience working with chronically homeless individuals with mental illness preferred. Must be eligible for a Washington State Department of Health Agency Affiliated Counselor Registration credential.
Job Conditions
While some work is performed in an office setting, this is a very physically active position, with most of the work performed in the community, including meeting with clients in the community, libraries, in the street and in encampments. Must be able to go into client’s residences and encampments, which may require climbing stairs and walking on uneven terrain. Incumbents must be able to bend, twist, squat, reach overhead, and lift 20 pounds. Must be able to speak and hear at normal conversational levels.
