PATH (People Assisting The Homeless) is urgently seeking candidates with case management or street outreach experience to join our South Bay Intensive Street Team as an Outreach Case Manager.

This position works primarily in the field throughout LA County's Service Planning Area (SPA) 8 with an assigned home office of our location in Long Beach. Cities encompassed by SPA 8 include: Athens, Avalon, Carson, Catalina Island, El Segundo, Gardena, Harbor City, Hawthorne, Inglewood, Lawndale, Lennox, Hermosa Beach, Manhattan Beach, Palos Verdes Estates, Rancho Dominguez, Rancho Palos Verdes, Redondo Beach, Rolling Hills, Rolling Hills Estates, San Pedro, Torrance, and Wilmington.

ABOUT THE PROGRAM

PATH's South Bay Street Outreach Program utilizes our Street Outreach Integrated Care approach to provide outreach, engagement and case management services to quickly assess and link clients to long-term housing, health, mental health and other supportive services. Services are delivered by the South Bay Intensive Street team that is comprised of case managers, substance abuse and mental health specialists, and medical providers.

WHAT IS AN OUTREACH CASE MANAGER?

As part of the South Bay Intensive Street Team, the Outreach Case Manager participates in daily street outreach and serves as the central point of contact through with care and services are coordinated during the period of time that clients are living on the street. Responsibilities of the Street Based Intensive Case Manager include:
• Providing field-based case management services including, but not limited to, independent living skills, housing stabilization, community integration, employment linkage, benefits establishment, and linkage to community providers for substance abuse, primary and mental health care
• Providing ongoing strengths-based case management support and service coordination to assist clients in obtaining and maintaining stable housing
• Collaborating with clients to develop individualized treatment plans that address short- and long-term goals
• Maintaining documentation standards set forth by the program contact and PATH policies

WHAT YOU BRING

We're looking for candidates that:
• Maintain and execute confidential information according to HIPAA standards
• Demonstrate knowledge or experience with case management techniques, including critical time intervention, harm reduction strategies, crisis intervention techniques and motivational interviewing.
• Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
• Work just as well independently as they do on a team
• Exercise mature judgement, and are highly motivated, self-starting and proactive
• Are excellent at communicating, whether in writing or verbally
• Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment

MINIMUM QUALIFICATIONS
• Hold a Bachelor's Degree in a related field OR have equivalent combination of education and experience

MANDATORY REQUIREMENTS

For this role, candidates must:
• Be able and willing to work flexible hours which may include evenings or weekends
• Have employment eligibility verification
• Have or be able and willing to obtain CPR/First Aid training
• Provide proof of full COVID-19 vaccination
• Successfully complete the following as a condition of hire:
• Tuberculosis Test
• Background Screening
• Drug Test

DRIVING

Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:
• Have reliable transportation
• A valid driver's license
• Proof of insurance and ownership for personal vehicles used during work duties
• The ability to qualify for PATH's insurance coverage

COVID-19 VACCINATION

Effective 11/01/2021 ll require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.
