Homeless Mental Health Support Coordinator

Permanent, Full Time (37.5 hpw)

We have a fabulous job opportunity for you to join our awesome team as a Support Coordinator! We support our brilliant customers who have goals to help their mental health and need support to gain more independence.
They live in a supported housing service in their own fully self-contained flat and have access to communal facilities within the service. We also have two dispersed properties in the local community. We help our customers achieve goals around their housing and other (often complex) support needs. We do this by working in a person-centred way to help promote independent tenancy skills and their health, wellbeing and resilience.
You’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support... planning, goal setting and regular reviews. You’ll create bespoke and personalised support plans with and for your customers. Each plan will look to meet our customer’s individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements.
You’ll also have responsibility for coordinating our support workers, apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.
We empower our customers to develop the independent living skills they need, to successfully move on. Together we’ll make a difference!
Working hours
You’ll work on a rota basis between the hours of 8am and 8pm Monday to Friday.
We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you.
About you
You’re passionate and motivated to help vulnerable people make positive and lasting changes to their lives. You’ll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.
To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe!)
To make sure our customers are well supported, it’s important that you have worked with vulnerable adults. Don’t worry, we won’t drop you in the deep end, we’ll help to you understand how things work at Home Group!
It would be amazing if you have worked in a housing environment before however it’s your “can do” positive attitude and enthusiasm that are critical to make this role a success. We can nurture your talent, just let us know in your cover letter why you have a passion for working here!
As this is a community based job, you may need to travel around to spend time with your customers, this may be using public transport, or if you drive and have access to a vehicle that would also be amazing! (we pay your expenses to cover this use!)
When you join, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one.

You’ll be working alongside our fabulous team who are super passionate about supporting customers to reach their goals and celebrating their achievements. To us we’re not just a team we’re a ‘work family’. Our senior manager Steph has been with Home Group 10 years! Angie who has worked in supported housing forever is currently stepping up as our awesome CSM and is waiting for the right candidates to add to our Juniper family.
At the service you’ll meet staff who have a wealth of Home Group knowledge including Stephen and Moses who work nightshifts and are committed to our values. We also have our Support Coordinator Neisha who is often there to order our lunch, alongside Mercy our support worker and June our day time concierge who both love being involved in our customer activities.
