Social Worker: Home and Community

Oxford & Norfolk County

The Social Worker works in the home care setting, in person and virtual visits, as part of the CBI Home Health team serving the Oxford and Norfolk community.

What you need to be successful
• You have a Bachelor of Social Work or a Master of Social Work
• You are registered as a member in good standing with the Ontario College of Social Workers and Social Service Workers
• You have the ability to travel within the Oxford and Norfolk community (Valid driver's license and insurance required)
• Your focus is to embrace person and family-centered care
• You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions
• You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
• You value the role a therapist plays in keeping clients at home when that is of personal importance
