Responsibilities
• Conduct nutrition assessments and develop nutrition care plans for clients
• Provide nutrition education and counseling to clients, including individual and group sessions
• Collaborate with other healthcare professionals to coordinate clients' care
• Monitor clients' progress and adjust nutrition plans as needed
• Conduct outreach and education activities to promote healthy eating and prevent nutrition-related health problems
• Maintain accurate and complete records of clients' nutrition assessments, care plans, and progress notes
• Participate in quality improvement activities to ensure the delivery of high-quality nutrition services
• Maintain knowledge of current research and trends in nutrition and dietetics
• Ensure compliance with all applicable regulations and standards of practice

Qualifications
• Must be a Registered Dietitian with current registration through the Commission on Dietetic Registration
• Minimum of 2 years of experience in clinical nutrition, preferably with older adults
• Experience with electronic health records and nutrition assessment tools
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of federal, state, and local regulations related to nutrition and dietetics
• Ability to maintain confidentiality and handle sensitive information appropriately
