Responsibilities

The Clinical Pharmacst is responsible for integrating patient-centered pharmaceutical care with traditional distributive and drug control activities. Provides consultative/clinical services including antibiotic dosing of vancomycin and aminoglycosides, initial warfarin dosing, renal dosing, IV-to-PO conversions, and parenteral nutrition. Requires strong clinical and skills for... providing drug information to nurses, physicians, and other health care providers. Capable of advising the patients' medical team on drug interactions, contraindications, and other relevant information to ensure proper, accurate, and optimal medication therapy. Responsible for adhering to and practicing within the guidelines of Georgia State Pharmacy Law.

Qualifications

Required Minimum Education:
B.S. from an accredited college of pharmacy

Preferred Education:
Pharm.D. from an accredited college of pharmacy
Completion of a PGY1 Pharmacy Residency

Required Minimum License:
Licensed by, and in good standing with, the Georgia State Board of Pharmacy or ability to obtain a Georgia
State Board of Pharmacy License within 6 months of hire date.

Preferred Certification:
BCPS

Preferred Experience:
Hospital pharmacy experience
Two years of clinical pharmacy experience
Tyrone GA USA
