The Health and Wellness Care Manager is responsible to lead innovative initiatives that will proactively assess, inform, and respond to the aging and health conditions of residents. This role works with residents and their families to ensure residents receive evidence-informed care to meet their changing health needs. This position will build Shalem’s capacity to care for, support, and provide appropriate resources to residents with dignity and compassion.
