Job Overview

To work as part of a nursing team to provide an early intervention and prevention nursing service for people who are homeless or rough sleeping across Somerset. This includes those in hostel or temporary accommodation.

Main duties of the job

You will work collaboratively with homeless as part of the Neighbourhood's team, working with multi- agency partners and local charities to provide a outreach nursing service to people in the community. When specialist services are required, you will act as the conduit to advocate and support people to access the care required.
