What does a Community Care Worker do at Hillingdon?

Our Community Care Workers play an important role in undertaking person centred Care Act assessments and help people to live as independently as possible. With initial support from your colleagues and managers, you’ll work closely with other professionals to achieve the best outcomes for the Adults in our Community, Hospitals and Intermediate Care settings.

You will be working in the Hospital Team within Adult Social Care.

The Hospital Team are a successful and energetic team, completing reassessments/reviews for older adults, and adults who have physical disabilities, and learning disabilities. We service the whole borough and also some reviews further afield. A smart allocation system is in place which maximises the results of our efforts and means that our targets are achieved. We pride ourselves on a supportive atmosphere to enable confidence growth and career development.

About You

We are looking for a Community Care Worker with the skill and commitment to successfully manage casework through to completion and is able to deal with cases, clients and issues with a degree of complexity. The job is equivalent to an unqualified Social Worker, and you will be carrying out face to face Care Act assessments with service users to determine if they meet the eligibility for services which range a package of care to a care home placement. This includes an understanding and application of Care Act 2014 legislation, Mental Capacity Act, personalisation, direct payments and managing risk. The role will require the completion of Mental Capacity Assessments, Continuing Healthcare Assessments with Health colleagues, and participation in multi-disciplinary meetings with external and internal partners.

Working within a very supportive and ever developing team you’ll be self motivated, enthusiastic, flexible in approach and able to work at a fast pace to meet team and departmental performance targets. You will have a good working knowledge of Social Services and its relevant legislation coupled with an understanding of the needs of people with disabilities. IT skills and ability to adapt to new computer processes are ideal to be able to manage your workload.

You will also be a flexible member of staff with excellent communication skills and good report writing ability and crucially, have the aptitude to have accountability for decision-making and use your own initiative to act decisively under pressure.
