Description:
The Street Medicine Team is an interdisciplinary team committed to providing high-quality, integrated primary and behavioral healthcare and social services. This interdisciplinary team works with individuals experiencing unsheltered homelessness in Atlanta, GA, providing street outreach and care. The nurse provides screenings, education, care coordination, wound care, and support for the interdisciplinary team.
Under the supervision of the Street Medicine Manager, provides clinical direction of paraprofessional personnel in an outpatient field environment serving clientele consisting primarily of uninsured persons, homeless persons and/or immigrants. Nurse will oversee Vaccine Program, maintaining accurate inventory counts and ensuring compliance with the vaccine management protocols. Is responsible for conducting health screenings, education, care coordination, wound care,and referrals for medical services. Health screenings include blood pressure, glucose, and flu shots. Obtains medical history and evaluates the overall health status of the individual. Collaborates with Medical Referral Specialist and makes referrals to Mercy Care and other appropriate healthcare providers. Participates as a team member in the patient’s plan of care. Provides support and acts as a resource for physicians and other healthcare providers as necessary. Responsible for maintaining and ordering medical supplies and materials under the guidance of the Street Medicine Manager. Performs light administrative duties, such as copying and filing. Complies with department policies and procedures, and supports the Mercy Philosophy. Participates in health fairs or community events on weekends as requested 3-4 times a year.
• Provides patient care and/ or support activities appropriate to ages served; primarily adults ages 18-65 or geriatric patients ages over 65.
EDUCATION REQUIREMENTS: Associate’s Degree or Bachelor’s Degree in Nursing. CPR/BLS certified. RN preferred.
EXPERIENCE REQUIREMENTS: At least one year related experience preferably in an outpatient care setting or outpatient education program. Desire to work with underserved and/or disadvantaged populations.
JOB KNOWLEDGE: Ability to effectively assess and evaluate patients, to make and implement plans. Knowledge of adult and adolescent healthcare, patient education, diabetes management principles, and home care continuum. Knowledge of growth and development including the adult and children, ability to adapt teaching to the age of the learner, including, age 18-65, geriatric over age 65, low literacy populations, and patients with special care needs. Excellent communication skills to develop and teach programs.
