We are looking to recruit a Social Care Practitioner to join a busy Generic Field work team working across the North of South Derbyshire. The successful candidate will have experience of working in an Adult Social Care environment to provide assessment and support under the Care Act 2014 to people with a variety of disabilities and care and support needs. To be able to demonstrate an ability to work in a person centred and strengths-based approach to achieve the ideal outcomes with people will be an asset. The candidate will need to have an awareness of the current legislation, policy and procedures that underpin good practice. The role includes the assessment and provision of equipment to support independence and safety in the home.

The successful candidate will contribute to the team’s responsibility to ensuring close working with partners in statutory and voluntary organisations to plan safe and timely discharges from acute and community hospitals.
