Position Highlights
• Identify through communication with patients/residents, their family members/significant others, and nursing center staff, patient/resident's strengths, social, emotional, and mental health needs' and then provide, develop, and/or aide them in accessing services to meet their needs.
• Collaborate with the interdisciplinary team to promote and protect resident rights and the well-being of all patients/residents. This includes educating staff on the role of the social services.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information so as to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community based agencies/services.
