System 1 training required. Main Duties & Responsibilities To be responsible for the provision of Occupational Therapy and Physiotherapy ensuring that services support the discharge of patients by providing specialist assessment and discharge planning. * To hold responsibility for own caseload, working with and without direct supervision * To work as part of a multidisciplinary team to facilitate timely assessment and timely, coordinated discharge planning "1) Registered with HCPC 2) Experience working in the community 3) Experience working with long term conditions, frailty, individuals with care needs.
