The Social Work Assistant will be responsible for receiving and processing applications of admission into The Salvation Army Peacehaven Nursing Home. He/She will also be required to provide administrative support and coordination to the Social Work department on new admissions.

Key Responsibilities
• Coordinate and setting up weekly/monthly Multidisciplinary Team Meetings (MDM)
• Prepare documents for financial assistance applications
• Collate statistics and submit monthly reports
• Conduct and follow up in means testing
• Coordinate home assessments and visits where necessary
• Post discharge follow up
• Organise events
• Attend to enquiries
• Assist in admin work

Job Requirements
• GCE ‘O’ Level/Diploma
• Experience in Social Work, especially in healthcare industry would be an added advantage
• On-the-Job Training will be provided
• Proficiency in Microsoft applications
• Highly organised, independent with good time... management skills
• Good interpersonal skills, accountable and responsible attitude
• Possess positive working attitude and ability to work with minimal supervision

Working Hours 5 days per week, Monday to Friday 8am - 5.15pm or 8.30am - 5.45pm
