Nurse Liaison Responsibilities


Our Hospital Liaison acts as a link to coordiante hospice care in the hospital and support discharge home with hospice.
Work closely with hospital case managers, palliative care team, physicians, and direct care hospital colleagues.
Build strong collaborative relationships with hospice admission team.
Educate patients and families regarding hospice care in and out of the hospital to create a hospice care plan individualized the patient and family's needs and wishes.
Admit hospice-eligible patients referred by hospital care team.
Review hospital referrals and proactively work with hospital colleagues to identify hospice appropriate patients, continuously modifying strategies and plans to ensure quality outcomes for hospitalized hospice patients.
Prepare and present regular reports of patient referrals, identifying trends, additional referral opportunities and obstacles to retention and growth of relationship between hospital and hospice teams.
Performs other tasks as assigned and within scope of practice.
Complete accurate and timely documentation of patient/family services provided.
Regularly attend IDG meetings to review newly admitted hospice patients.


Nurse Liaison Requirements


Registered Nurse with active, unrestricted license in the State of Michigan
Experience in a Hospice Nursing or Hospital Case Manager role (preferred), or experience in end-of-life care, community health nursing, acute care or emergency department nursing
Reliable transportation with a valid state driver's license and current car insurance
