Whether for short-term rehabilitation or long-term care, we ensure every individual receives the attention and resources they need to achieve their highest level of independence and well-being.Are you passionate about promoting healthy nutrition and wellness for individuals?As a Dietitian, you will play a critical role in ensuring the nutritional needs of our residents are met and that they receive appropriate care based on their individual health conditions and dietary requirements.Key Responsibilities:• Nutrition Assessment: Evaluate the nutritional needs of residents through assessments and collaborate with healthcare providers to develop personalized dietary plans.• Meal Planning: Develop and implement nutritious, balanced meal plans tailored to individual medical needs, preferences, and dietary restrictions.• Education and Support: Educate residents, families, and staff about the importance of nutrition in maintaining health and preventing disease.• Dietary Monitoring: Monitor residents' nutritional intake and make adjustments as necessary, ensuring their ongoing health and comfort.• Collaborative Care: Work with other healthcare professionals to address residents' overall health needs, adjusting dietary plans as part of an interdisciplinary care approach.• Compliance and Documentation: Ensure all meal planning and dietary services comply with state and federal regulations.
