About the Role:
• Work collaboratively with our interdisciplinary care team to develop and implement comprehensive patient-specific care plans
• Deliver high-quality skilled care to patients in their homes, focusing on results and compassionate care delivery
• Thoroughly document care delivery daily in our electronic medical record (EMR) system
• Participate in ongoing education and training to stay up-to-date on industry best practices and regulatory requirements
Requirements:
• Current Vocational/Practical Nurse License in good standing
• Valid state driver's license and auto liability insurance
• One year of experience as a Licensed Vocational/Practical Nurse in a clinical care setting, with home health experience preferred
