Home Health Physical Therapist
About the Role:
We are seeking a compassionate and skilled Physical Therapist to join our team of healthcare professionals in Auburn, Maine. As a Home Health PT, you will have the opportunity to make a meaningful impact on patients' lives by providing personalized therapy services in their homes.
The ideal candidate will hold a valid PT License in the State of New Hampshire and be passionate about delivering exceptional care to individuals struggling with strength and mobility challenges.
Key Responsibilities:
• Develop and implement customized therapy plans to meet patients' unique needs and goals
• Conduct assessments, evaluations, and treatment sessions in patients' homes
• Collaborate with healthcare teams to ensure seamless care coordination

Requirements:
• Valid Physical Therapist License in the State of Maine
