Position Summary:

Care Hawaii is a behavioral health care organization seeking a compassionate and dependable Team Lead to support the residential and groups home programs.

Residential Services contracts with the State of Hawaii for adult mental health services. The Team Lead is a member of a multidisciplinary team, responsible for carrying out an array of services according to his/her/their training and discipline. Team Leader is responsible for overseeing the SRSP group home. Operational functions may include but are not limited to hiring and training personnel, program financial management, managing the schedule, ensuring all health and safety requirements are met, staff credentialing is maintained, and that referrals, authorizations, and billing are processed The Team Leader functions under the direct supervision of the Program Director. The Team Leader has direct responsibility for all SRSP staff on Oahu. Residential locations are on the islands of Oahu and Hawaii.

Position Qualifications:

· Minimum Education: Must meet the educational requirements of a Team Leader as required by contracting and/or monitoring agency AMHD/CCS; Preferred to be a Qualified Mental Health Professional (QMHP) licensed in the State of Hawaii in one of the following fields: Licensed Clinical Social Work, Licensed Marriage and Family Therapist, Licensed Psychologist, Licensed Psychiatrist, or Advanced Practice Registered Nurse in Behavioral Health, Licensed Registered Nurse in the State of Hawaii or Master's degree, Bachelors or Associate degree in Behavioral Science, Social Sciences or Human Services and have two (2) years' experience, in mental health required. If the team leader is a MHP, the residential team leader shall be supervised by the Provider's QMHP, Exceptions made for team leaders pending their license.

· Minimum Field-of-Experience: Strong clinical skills and experience demonstrating competence in problem-solving, critical thinking, collaboration, and clinical competencies in providing education and training to staff members. Computer proficiency in various software.

· Preferred Experience: Critical thinking, leadership ability, and demonstrating excellence in interpersonal skills to effectively work with all levels of personnel and with other agencies. Interest and knowledge of current trends and new developments in the mental health field for the adult target population.

· Required Certification/Registration:

o CPR

o First Aid

o CPI

o TB (2-step upon hire)

o Physical Exam (Pre-employment & Annually)

Job Competencies

· Required Skills: Mature, dependable, and responsible. Must be comfortable working with adults with a history of severe mental illness and their families. Must be culturally sensitive and aware. Must know the basics of ethics, HIPPA standards, and problem

solving. Must display sound judgment, a decent work ethic, and personal integrity. A working knowledge of diverse populations including treatment of behavior disorders, mood disorder, anxiety disorders and Schizophrenia/Schizoaffective disorder.

· Technical Skills: Basic use of Microsoft Word Suite, fax machine, telephone, DVD player, and television

· Working Environment: Able to work indoors and/or outdoors in a well vented or air-conditioned area, possibility of visiting and/or working at other residential sites. Exposure to infectious disease and/or viruses is a risk. The noise level in the environment is usually moderate.

· Special Demands: Ability to respond effectively to the varied needs and demands of consumers, families/significant others, staff, physicians, and other multidisciplinary team members. Must have a strong commitment to the right and ability of each person with a severe mental illness to live in normal community residence, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and services.

· Consumers Served: Employee will service adults diagnosed with a serious mental illness (SMI) and substance use disorders, who are at least 18 years of age. Consequently, the employee must be competent in the interpretation of client's self-report or behavior. The treatment of clients should reflect an understanding of the developmental needs and range of treatment needs for each client.

· Physical Demands:

1. Assist consumers who have physical disabilities such as those who are wheelchair bound, willingness to learn body mechanics.

2. Ability to assist with direct consumer care; ability to communicate effectively both verbally and in writing is essential.

3. Ability to follow instructions and motivation to facilitate various rehabilitative groups. Frequent reading of printed materials is essential.

4. Listening skills necessary to effectively respond to fellow co-workers, associates, physicians, and clients.

5. Ability to move about throughout the shift to assist clients in meal preparation, various chores, and AOL's.

Duties and Responsibilities:

· Oversee the SRSP staff on Oahu.

· Complete intake assessments and treatment plans within seven (7) days of admission and every six (6) months.

· Well-versed in the delivery of SRSP and Group Home services.

· Ensure staff receives supervision on a monthly basis (minimum) but may occur more often as needed.

· Oversees the overall clinical operation of the program on-site at each residence.

· Complete LOCUS.

· Responsible for authorization of service and coordination with AMHD.

· Shall provide clinical consultation and training to direct care providers as needed.

· Provides documentation and reports as required.

· Assists assigned Programs to meet credentialing, licensing and contract requirements.

· Participates and documents QA Activities.

· Fosters good relationships among care providers; maintains dialogue in interaction with other professionals to identify and resolve existing problems, as well as to explore new avenues for service; participates/contributes in meetings pertinent to area of responsibility; promotes quality client care through a spirit of mutual cooperation with other staff

· Works with staff to establish appropriate and ongoing process and outcome indicators, and periodic program evaluation studies; demonstrates improvement where needed

· Attends trainings/meetings as assigned

· Assists Clinical Supervisor to monitor, audit, and ensure contract compliance for all program services. Become thoroughly familiar with all residential contracts and notify the Clinical Supervisor of any changes or modifications as they happen.

· Collects necessary CARE Hawaii and AMHD/CCS data and prepare monthly QI reports and quarterly QA, CQRT reports, surveys, type letters, memos, and general correspondence.

· Processes client referrals: verify consumer referrals packets are complete (i.e., all required documentation is current and submitted) before they are given to the Clinical Supervisor. Establish prioritized consumer waitlist for housing admissions. Coordinate client transfer to other facilities according to established procedures as needed.

· Comply with AMHD Utilization Management, CCS, Ohana, and any other policies and procedures regarding authorization requests, and compensation & payment schedule, per contracts.

· Requesting of Admit, Continued Stay, and Discharge Authorizations for SRSP per contract.

· Assist in tracking of authorizations from AMHD/CCS UM by inputting authorizations into the database.

· Faxing and tracking behavioral health service (BHS) requests for all residential programs, per contract.

· Facilitates consumer admissions, discharges, and treatment planning.

· Development of program plans to meet consumer's needs by exchanging information with staff and reviewing collected data.

· Adheres to federal and state laws and regulations as well as company and agency policies and procedures regarding compliance, labor, health and safety issues.

· Coordinate with program nursing staff for monitoring of client charts/medical records. Maintains and inspects all consumer charts to ensure compliance with company policy and contract compliance.

· Monitor staffing schedules for all residential services to ensure all shifts are covered and will provide shift coverage (i.e., day/evening/NOC) when necessary.

· Supervise professional staff by interviewing and hiring, assigning and reviewing work, training employees, resolving non-clinical work problems, and evaluating the performance of incumbents.

· Demonstrates self-evaluation skills and takes responsibility for and appropriate action to remedy or reduce limitation; independently seeks formal and informal professional development activities to ensure the highest standard of knowledge and skill.

· Management of consumer's records.

· Monitor and track all staff credentials to ensure we follow AMHD, CCS and CARE requirements.

· Collect necessary monthly data such as (but not limited to): QI, QA, CQRT, QOLI reports, and consumer satisfaction surveys; type letters, memos, and general correspondence.

· Fosters good relationships among employees and subcontractors; maintains dialogue in interaction with other professionals to identify and resolve existing problems.

· Conduct staff meetings with team members to determine residential progress and to develop a new or modified programs.

· Monitor all AMHD/CCS contractual monthly staff trainings, trainings/fire, emergency drills for SRSP are conducted, tracked and logged monthly.

· Conduct monthly audits at each residential facility and ensure all sites are following AMHD/CCS Housing Quality Standards.

· Ensure that all food services comply with the State of Hawaii, Department of Health's regulations and requirements.

· Monitor and process all submitted work orders.

· Collaborate with Registered Dietitian Nutritionist (RDN) to develop a menu that has a variety of options and is culturally sensitive by utilizing consumer feedback preferences.

· Conduct at least one hour of face-to-face instruction for SRSP on developing skills and/or activities to successfully re integrate into the community (e.g. shopping, cooking, budgeting and planning meals, understanding nutritional labels, and sanitary preparation, storing, and cleaning all in accordance with the DOH, Sanitation Branch).

· Monitor that each residential facility is labeling all perishable food products with expiration date, and that all meals are being separated and labeled in accordance with weekly menu) relative to number of clients at site as needed and portion/serving guidelines.

· Make appropriate menu item substitutions when an item is not available for purchase, and document on site menu.

· Document all food purchases on appropriate spreadsheet, maintain Dietary Binder at each site.

· Purchase all household supplies for SRSP group home, document, and monitor and track all monthly supply requests from each site on a spreadsheet.

· Collect monthly rent payments from SRSP group homes. Provide a rent receipt for each payment.

· Provide a late payment notice to each consumer, representative payee, and/or case manager monthly as necessary, document all rent payments on appropriate spreadsheet.

· Will act in the capacity as an MHT in case of a staff absence.

· Maintains a current Physical Evaluation, TB Clearance, CPI, First aid and CPR certification.
