In the role of Revisit LPN, you will apply your extensive training to offer personalized care to our patients in a fulfilling setting that encourages independence. This position will cover various towns, ensuring you make a difference in multiple communities.
• Facilitate communication between patients and the interdisciplinary team.
• Develop and nurture therapeutic relationships by providing education, promoting self-advocacy, and fostering independence.
• Utilize state-of-the-art technology to document the skilled services you provide.
Qualifications
• Must hold a valid CT LPN license.
• Requires one year of LPN experience; homecare experience preferred.
• Valid driver's license and reliable transportation are mandatory.
• Computer proficiency is required.
