Summary
• This position ensures the safe and effective use of medications by assessing for optimal drug selection, dosing regimen, and drug therapy monitoring services. The position will evaluate patient specific care plans and resolve medication related issues with the goal of optimizing pharmaceutical care. This position requires an understanding of the various work functions and activities associated with drug distribution, intravenous admixture services (compounded sterile products), pharmacy automation and the provision of drug information to the Medical, Nursing and Ancillary staffs, and the community in keeping with TJC, Department of Health and Senior Services, DEA/BNDD and Board of Pharmacy laws, rules and regulations.
• This position is employed by Mosaic Health System.
Duties
• Provide Pharmacist services to the Cancer Center including, but not limited to, compounding and dispensing chemotherapy, workflow prioritization, and patient education.
• Continuously evaluate oncology formulary, in coordination with pharmacy leadership, for optimal clinical effectiveness
• Maintain necessary medication inventory in Cancer Center under direction of pharmacy leadership
• Evaluate patient specific care plans and resolve medication related issues with the goal of optimizing pharmaceutical care
• Understands, performs and is compliant with all pharmacist steps in the Medication Use Process.
• Other duties as assigned
Qualifications
• All required education is a minimum requirement. Higher levels of education are acceptable. Bachelor's Degree required. OR PharmD from an accredited college of Pharmacy required.
• License in good standing with the Missouri State Board of Pharmacy required within 120 days of hire. MTS (Medication Therapy Services) Certificate from the Missouri State Board of Pharmacy required within 120 days of hire.
• Experience with medication distribution, IV Admixture programs, and in-service programs to public, healthcare professionals, and providing clinical pharmacy services required. Experience in computer applications to pharmacy services required. Experience in pre-packing\batch preparation\compounding skills required.
