Clinical Pharmacy Support: Job Responsibilities and Tasks
We are currently seeking a dedicated and detail-oriented individual to join our team as a Clinical Pharmacy Support team member. In this role, you will be responsible for providing essential support to our clinical pharmacy team, assisting with various tasks to ensure the safe and effective delivery of pharmaceutical care to our patients. The Clinical Pharmacy Support position is crucial in helping our pharmacy operate smoothly and efficiently, ultimately contributing to improved patient outcomes and satisfaction.
Responsibilities:
• Medication Reconciliation: Support the accurate and up-to-date management of medication records for patients entering the hospital or transitioning between care settings. Collaborate with healthcare providers to reconcile any discrepancies and ensure a smooth continuation of care.
• Patient Education: Deliver informative sessions to patients and caregivers on medication usage, including details on medication instructions, potential side effects, and adherence approaches. Address patient queries and apprehensions to encourage proper medication management.
• Gathering Medication Data: Acquire detailed medication histories from patients, caregivers, or digital health records, comprising prescribed medications, non-prescription remedies, and herbal products. Ensure precise documentation of medication histories to support clinical decision-making processes.
• Assistance with Prior Authorization: Help with the prior authorization procedures for specialty medications, facilitating collaboration between insurance companies, prescribers, and patients to obtain approval for necessary treatments. Ensure accurate completion of all required documentation and timely follow-up on pending authorizations.
• Maintain precise and thorough documentation of clinical interventions, medication therapy management activities, and patient interactions within electronic health records (EHR) or pharmacy systems to comply with documentation standards and regulations.
• Medication Delivery Coordination involves overseeing the coordination of medication delivery services for discharged patients, ensuring that medications are dispensed accurately and in a timely manner to patients' homes or preferred pharmacy locations. Effective communication with patients, caregivers, and pharmacy personnel is paramount for facilitating a seamless medication transition.
• Collaborate on inventory maintenance by assisting with medication orders, restocking inventory levels, and monitoring expiration dates. Maintain pharmacy storage areas in an efficient manner, adhering to storage guidelines for proper handling of medications and supplies.
Requirements:
• Meeting the requirement of a high school diploma or its equivalent credential is essential. It is beneficial, but not mandatory, to hold an associate degree or to have wrapped up a pharmacy technician training program.
• Although being certified as a pharmacy technician (CPhT) is preferred, it is not mandatory. Prospective employees should be willing to work towards certification within a specified timeframe after commencing their employment.
• Experience: It is advantageous to have prior experience in a pharmacy or healthcare setting. Understanding pharmacy operations, medication terminology, and basic pharmacology concepts is beneficial.
• Demonstrated proficiency in using pharmacy software systems, electronic health record platforms, and other technology tools commonly utilized in pharmacy operations. Skilled in quickly learning and adapting to new software and technology interfaces.
• Interpersonal Skills: Proficient in communication and customer service. Ability to interact professionally with patients, caregivers, healthcare providers, and pharmacy team members.
