We are looking for Full-Time and Per Diem Home Health Physical Therapists at GrandCare Health Services to cover the following Central, East and South San Diego County areas:
• South County: National City, Chula Vista, Imperial Beach, Coronado and surrounding areas
Under the direction of the Regional Director of Rehabilitation Services, the Physical Therapist (PT) restores the patient's function, alleviates pain, and prevents disabilities by planning and administering medically prescribed physical therapy.
Duties and Responsibilities:
• Meet the patient's goals and needs and provide quality care by assessing and interpreting evaluations and test results, determining physical therapy treatment plans in consultation with physicians or by prescription
• Help patients accomplish treatment plan and accept therapeutic devices by administering manual exercises, instructing, encouraging, and assisting patients in performing physical activities, such as non-manual exercises, ambulatory functional activities, and daily-living activities and in using assistive and supportive devices, such as crutches, canes, and prostheses
• Evaluate effects of physical therapy treatments and fit of prosthetic and orthotic devices by observing, noting, and evaluating patient's progress, recommending adjustments and modifications
• Complete the discharge plan by consulting with physicians, nurses, social workers, and other health care workers, contributing to patient care conferences
• Continue therapeutic plan following discharge by instructing patients, families, and caregivers in-home exercise programs, recommending and/or providing assistive equipment
• Document patient care services by charting in patient and department records
• Maintain patient confidence and protects the company’s operations by keeping information confidential
• Maintain a safe and clean working environment by complying with procedures, rules, and regulations
• Protect patients and employees by adhering to infection-control policies and protocols
• Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies
• Develop physical therapy staff by providing information, developing and conducting in-service training programs
• Comply with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions
• Contributes to team effort by accomplishing related results as needed
• Perform related duties as assigned
Qualifications and Requirements:
Continuing Education Requirements:
Agency personnel is expected to participate in appropriate continuing education as may be requested by their immediate supervisor. In addition, agency personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. All agency personnel must attend mandatory educational programs.
Experience:
• One (1) or more years of experience as a PT;
• Home Health experience is required;
• Experience with OASIS is required.
