The Role
Under general direction of the RN and/or Physician, the Licensed Practical Nurse-LPN will support providers in provision of care including recording vital signs, discussing chief health concerns, performing testing, assisting with procedures and documentation. The incumbent will assess, plan, evaluate, and deliver nursing care in collaboration with RN and/or Physician. This work is carried out in support of the mission and goals of Samaritan Daytop Village.
What You Will Do
• Assist with patient care including checking vital signs, blood pressure, pulse rate, breathing, temperature, weight, discussing chief health concerns, administers injections and performs test.
• Take patient’s medical history, all pretreatment formalities and enters data in computer system.
• Follow orders of physician and registered nurse in providing care.
• Record in patients’ record a detailed report of care given.
• Handle proper charting, documentation and maintenance of patients’ medical records.
• Check, clean and dress wounds.
• Reinforce instructions given to patient by RN or Physician.
• Notify RN or Physician of significant findings or changes in patient’s physical or mental condition.
• Place phone calls to pharmacies to order prescriptions.
• Maintain confidentiality of patient records.
• Pull and prepare patient charts.
• Maintain neatness of work area.
• Stock patient rooms with supplies.
• Provide emotional support as needed.
• Initiate emergency assistance as per policies and procedures.
• Attend staff development trainings and nursing conferences.
• Keep skills current and increase knowledge and productivity.
• Perform other duties as requested.
Who You Will Be
• NYS LPN licensure.
• BLS certification.
• High School or GED.
• Graduate of a formal Technical or Vocational LPN Program.
• At least One (1) year experience as an LPN working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI).
• Available to work a flexible schedule as needed in response to participant and staff needs.
• Computer literacy including proficiency in Microsoft Office Suite and EHR.
• Ability to put patients at ease and explain physician’s instructions.
• Ability to multi-task with excellent organization skills.
• Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
• Knowledge of medical terminology.
• Team player and ability to work independently.
