• Job Title:** Registered Nurse (RN)
• Home Health
• Job Specialty:** Home Health
• Job Duration:** 13 Weeks
• Shift:** 8-hour days, Monday to Friday, 8:30 AM
• 5:00 PM, plus one weekend every four weeks
• Guaranteed Hours:** 40 hours per week
• Experience:** Minimum of 2 years as an RN, with at least 1 year in Home Health within the last 2 years
• License:** RN license in CA
• Certifications:** Basic Life Support (BLS)
• Must-Have:**
• Experience with EMR systems, particularly Home Care Home Base (HCHB) is strongly preferred
• Job Description:**
• Deliver nursing care in a home health setting.
• Complete modules required before the start of the assignment (3-4 hours, not billable).
• Provide exceptional patient care, adhering to care plans to meet patient needs.
• Work independently to deliver quality care to patients in their homes.
• Requires two supervisory references, including one from the past year and another from the last three years.
• Commit to a weekend shift every four weeks.
• Participate in patient charting and documentation through the HomeCare Home Base system.
• Mileage reimbursement according to GSA guidelines.
• Be part of post-acute nursing care.
• Hospital may cancel up to three times per contract, which may include full shifts or partial shifts.
