Position Description
Provides personal care services in accordance to the Individual Support Plan (ISP). The Direct Support Professional is assigned to specific Individuals and performs services for Individuals, as necessary, according to their ISP to maintain their personal comfort, promote independence within the community and achieve an everyday life.
Essential Job Functions/Responsibilities
• Performs simple procedures according to Individual Service Plan
• Assists individuals with daily living skills and community skills
• Utilizes Person Centered Practices to assess individual needs
• Completes training with individuals towards achieving ISP goals
• Performs personal care activities, including but not limited to:
• Bathing
• Shampooing/hair care
• Skin care/nail care
• Oral hygiene
• Shaving
• Dressing
• Toileting/elimination
• Catheter care
• Provides support in areas of independent/home-living such as:
• Meal preparation/feeding/fluid intake
• Provides assistance in in the individuals laundry
• Light housekeeping in areas directly related to the individual’s living area
• Shopping as related to the individual’s community needs and preferences
• Reports any observed or reported changes in the individual’s condition and/or needs to the Executive Director or designee
• Documents care provided and completes daily progress notes. Completes the appropriate records to document care provided. Submits documentation at least weekly
• Promotes personal safety and a safe environment for individuals by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Executive Director or designee
• Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs
• Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested
• Maintains confidentiality in all aspects of the job
• Attends office required programs to meet compliance requirements. Identifies learning needs to supervisor
• Performs other related duties and responsibilities as deemed necessary
Job Types: Full-time, Part-time
Salary: $16.00 - $18.00 per hour
