Qualifications: Bachelor of Science Degree in Occupational Therapy from and accredited program. Successful completion of National Certification Examination for OTR. Assumes responsibility for ongoing continuing education and professional development. Current state licensure or license eligible.
Essential Job Functions
As a Licensed Occupational Therapist in a Grace Management Services facility you must be able to perform the following essential functions as demonstrated by the ability to:
Provide occupational therapy services to residents and initiates referrals when appropriate and in accordance with Occupational Therapy Department Procedures.
Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
Evaluate residents to obtain data necessary for treatment planning and implementation and conducts specialized evaluations as indicated.
Interpret and communicate evaluation findings to residents, families and team members.
Establish individual resident schedules in conjunction with other team members.
Develop treatment plans by identifying a target list, including short and long-term goals and methods to achieve identified goals.
Coordinate treatment plan with resident, family and other team members.
Implement and conduct treatment as outlined in treatment plan.
Monitor resident’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
Develop appropriate home or programming within the community to maintain and enhance the performance of the resident in their own environment.
Document results of resident’s assessment, treatment, follow-up and termination of services.
Adapt treatment to meet the individual needs of the resident.
Provide, recommend and fabricate adaptive devices or other equipment.
Train resident, family and appropriate staff in the use or application of such equipment.
Participate in discharge planning.
Supervise and directs Certified Occupational Therapy Assistants (COTA) in the implementation of the treatment plan.
Supervise Rehabilitation Aides, students and volunteers.
Review the quality and appropriateness of the total services delivered and individual occupational therapy programs for effectiveness and efficiency, using predetermined criteria.
Participate in resident, family and staff education.
Confer formally and informally with other team members in coordinating the total rehabilitation program of the resident.
Adhere to Occupational Therapy Department procedures regarding documentation of occupational therapy services.
Provide in-service education to the members of the resident’s care team and education to the community.
Provide treatment in accordance with established standards of practice, department procedures and productivity standards.
Consistently demonstrate sound judgment in the evaluation, planning, implementation and follow-up of resident therapy programs.
Report to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned shift.
Assist with and participates in Quality Assurance Program as required.
Maintain a current knowledge of the law and regulations regarding rehabilitative services and consults with supervisor to ensure that all employment actions are in compliance with applicable regulations.
Attend and participate in scheduled training, educational classes and meetings required or requested for position.
Ensure a safe environment and utilizes tools and equipment in a safe manner complying with all infection control, universal precautions and OSHA standards for the healthcare professional.
Report all hazardous conditions including incidents, injuries and equipment to Supervisor/appropriate personnel immediately.
Respond to and acts appropriately in emergency or disaster situations.
Adhere to high standards of cleanliness, grooming, hygiene and dress code.
Adhere to all policies of company, including job description, certification requirements, mission statement, HIPAA, confidentiality standards and resident’s rights.
Adapt to changes in responsibility in a positive manner, is receptive to new ways of completing tasks and in flexible in carrying out assignments.
Communicate and interact professionally and effectively with residents and coworkers throughout the community ensuring a cooperative environment while maintaining self-control.
Communicate and interact professionally and effectively with family members in attending to and meeting their requests.
Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the organization.
