Purpose And Scope:

The Clinical Advancement Program (CAP) incorporates the Ambulatory Care Nurse, a registered nurse who is accountable for providing and coordinating clinically competent care to a particular group of patients. They assess, plan, implement, and evaluate patient care, as well as delegate tasks. As a member of the ESKD healthcare team, they participate in decision-making... teaching, leadership functions, and quality improvement activities, all of which aim to enhance patient care outcomes and facility operations.

Principal Duties And Responsibilities:

To adhere to established nursing practice standards, it is imperative to operate within Fresenius Kidney Cares framework of policies and procedures while considering relevant state and federal regulations.
• Performs all vital tasks while working with the guidance of the Clinical Educator, Preceptor, or other Registered Nurses and following the directives given by the Supervisor.
• The healthcare professional responsible for a patient's hemodialysis treatment performs a continuous and methodical collection and examination of patient data, monitoring for overall treatment efficacy. This data is recorded in the patient medical record, and the treatment plans are adjusted and modified as necessary. Physicians or other medical personnel are informed about any modifications to the patient's treatment plan.
• Evaluates, cooperates, and notes the primary learning needs of patients and their relatives, ensuring to offer the best possible education service throughout the patient's care.
• Supervises the administration of hemodialysis therapy to assigned patients collaboratively with direct and indirect patient care staff, ensuring the safe and effective provision of treatment.
• Dispenses medication as directed or in compliance with accepted algorithms, while also documenting the correct medical reasoning and the effectiveness of the drug.
• Supports or takes part in implementing measures for emergency management.
• Serves as an essential component of the healthcare team, participates in staff instruction and helps in the orientation process for new members as provided by direction.
• Confirms that the appropriate procedures for laboratory collection, processing, and shipping are adhered to and schedules any missed or incomplete laboratory collections to be rescheduled.
• The Interdisciplinary Team can help identify expected outcomes for assigned patients, while facilitating the documentation and updating of nursing assessments and plans of care.
• Ensures that patients are aware of the different treatment pathways, including transplantation.
• Completing the CAP requirements is essential to move ahead in your academic or professional career.
• Carries out other responsibilities as directed by the Manager.

• Regular work duties involve desk work, utilizing computer equipment, conversing with patients, medical professionals, and other hospital/facility personnel.
• As part of this position, there may be a need to travel to different facilities or training centers.

One can accept the role of overseeing Patient Care Technicians/LPNs/LVNs/RNs as a Team Leader or designated Nurse in Charge after ensuring that all the prerequisites have been met:
• Completion of all FKC education and training requirements for new employees is crucial for their success and productivity in the workplace.
• It is mandatory for an RN applicant to have a minimum of 9 months of experience.
• Applicants for the registered nurse position are obliged to have gained expertise in both chronic and acute hemodialysis, which must be at least three months long.

Education and Licensure:
• A nursing program graduate from an authorized educational establishment.
• The valid state certification that is currently necessary for legal operation.
• A current certification in BLS CPR is an essential component of healthcare providers' competency and professionalism.
• The practice requirements of the state where the individual is employed must be met.

Experience And Required Skills:
• For those with less than one year of experience in nephrology nursing within the last 24 months, an opportunity exists to start at an entry-level RN position.
• Nine months of experience as a Registered Nurse are necessary to be preferred for this role.
• A preference will be given to those who have experience providing hemodialysis to patients with chronic and acute conditions.
