Purpose And Scope:

As a member of the Clinical Advancement Program (CAP), the Outpatient Care Nurse, a registered nurse, is responsible for providing and coordinating clinically competent care to their assigned group of patients. They assess, plan, implement, and evaluate patient care and delegate tasks as needed. Moreover, they work together with the ESKD healthcare team to participate in... decision-making, teaching, leadership functions, and quality improvement activities aimed at improving patient care outcomes and facility operations.

Principal Duties And Responsibilities:

Fully executing assigned duties and meeting responsibilities requires diligent adherence to Fresenius Kidney Cares policies, nursing practice standards, and state and federal regulations.
• Performs all necessary functions as instructed by the Supervisor while receiving guidance from the Clinical Educator, Preceptor, or in conjunction with other Registered Nurses.
• A thorough and methodical collection and analysis of patient data are carried out during hemodialysis treatment. The healthcare professional assigned to the patient makes modifications to the treatment plan as necessary after documenting the information in the patient's medical record. When appropriate, medical personnel are notified of the changes made.
• Arranges, works in conjunction with, and takes note of the patients and their families' basic learning requirements, aiming to offer the initial and ongoing education accordingly.
• Oversees the daily execution of hemodialysis therapy for assigned patients along with direct and ancillary patient care staff, ensuring the delivery of safe and effective care.
• Provides medication in line with the given prescription or approved algorithm(s), and keeps a record of the appropriate medical explanation and the outcome.
• Takes part or supports in implementing measures for responding to emergencies.
• Functions as a useful tool for the healthcare team, takes part in the training of personnel and aids in the induction of new members as instructed.
• Ensures that laboratory collection, processing, and shipping procedures are carried out accurately while rescheduling any missed or inadequate laboratory collections.
• A coordinated effort among the Interdisciplinary Team aids in the identification of expected outcomes and documentation of updated nursing assessments and plans of care for assigned patients.
• Takes responsibility for educating patients about transplant and treatment motilities that can be utilized.
• Failure to fulfill the CAP requirements will impede your progress and may prevent you from advancing.
• Performs any other tasks assigned by the Administrator.

Physical Demands And Working Conditions:

• The responsibilities of this role may require performing similar tasks in other locations with the same level of physical requirements and work environment mentioned earlier.

One can become a team leader or a designated Nurse in Charge to administer Patient Care Technicians/LPNs/LVNs/RNs after meeting all the necessary requirements:
• New employees who have successfully completed all FKC education and training requirements are better equipped to face challenges in their role.
• To apply for the RN position, candidates must possess at least 9 months of experience.
• Any registered nurse seeking this job must fulfill the requirement of having, at the very least, 90 days of experience in both chronic and acute hemodialysis.

Education and Licensure:
• Someone who accomplished a nursing curriculum approved by an accredited organization.
• The present state validation of educational and professional requirements.
• Certification in BLS CPR is requisite for healthcare practitioners to demonstrate their competence in emergency response.
• Adhering to the practice necessities of the state of one's employment is crucial.

Experience And Required Skills:
• There is an opening for RNs with less than one year of recent nephrology nursing experience to join the team.
• To be considered, the candidate should have worked as a Registered Nurse for a minimum of nine months (preferred).
• Candidates with a background in both chronic and acute hemodialysis will be highly considered.
