Job Description: Licensed Practical Nurse (LPN)

The Licensed Practical Nurse works within the framework of the Patient Care Services Organization as a member of the health care team. The LPN delivers patient care under the direction of a Registered Professional Nurse within the realm of the New York State Practice Act. The LPN gathers, and documents data related to physiologic, cognitive... cultural, and spiritual status, and assists in goal directed plans of care which are evidence-based and age specific. The LPN adheres to unit and human resources policies, participates in development and attainment of unit/service, and patient care goals, and organizes the delivery of patient care in an efficient and cost effective manner. The LPN assumes responsibility and accountability for professional standards and for own professional practice.

Qualification Requirements:
• Must be a graduate of an accredited School of Practical Nursing.
• Must hold current New York State registration or possess a limited permit to practice in the State of New York and current BLS certification.

Responsibilities:
• Assist patients in reaching outcomes as established by the RPN in the plan of care.
• Gather and document data related to physiologic, cognitive, cultural, and spiritual status.
• Implement care through utilization and adherence to established standards which define the structure, process, and desired patient outcomes of the nursing process.
• Communicate information needing referral to RPN and reinforce teaching plan established by RPN.
• Organize the delivery of patient care in an efficient and cost-effective manner.
• Maintain role-specific annual competency and recognize and communicate situations needing referral to an RPN.
• Relate to/assist patients, families, significant others, and coworkers with respect and kindness.
• Maintain confidentiality of information, demonstrate the knowledge and skills necessary to provide patient care appropriate to the age and special characteristics of the patients served, and perform data collection/documents interventions within the protocol or plan of care and objective patient care data on Daily System Assessment.
• Provide technical support as demonstrated by knowledge base and validation of competency skills related to medication administration, assisting and providing care to clients with peripheral IV Therapy only (no central line devices), performing phlebotomy, safety measures, personal hygiene skills, nutritional skills, elimination skills, heat and cold applications, admission/transfers/discharge, perioperative care, musculoskeletal skills, respiratory skills, transfer/transport skills, and specimen collection/testing.
• Assure Safety and Cleanliness of Environment, Equipment, and Supplies.

Requirements:

The LPN may not initiate Plans of care or Protocols, complete 24 hour analysis or End of Shift Summaries, or give end of shift report.

Ability to communicate cooperatively and effectively to patients, family members, employees, and others.
• Ability to document patient care on established forms.
• Ability to listen well, to take direction and to engage in interactive dialogues with others.
• Ability to seek out the input of others to achieve consensus.
• Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals, accurately with medication administration. Ability to compute rate, ratio, and percent.
• Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
• Ability to deal with problems involving several concrete variables in standardized situations.
