Job Description: Bushwick Center is seeking a motivated and experienced Sub Acute Unit Manager LPN to join our team. As the Sub Acute Unit Manager, you will play a pivotal role in overseeing the operations of our sub-acute unit, ensuring the delivery of high-quality care to our residents, and providing leadership and support to our nursing staff.

Responsibilities of Unit Manager:

Clinical Oversight:
• Provide clinical supervision and oversight to nursing staff on the sub-acute care unit, ensuring adherence to established policies, procedures, and regulations.

Patient Care Coordination:
• Coordinate and oversee the delivery of care to sub-acute patients, collaborating with the interdisciplinary team to develop and implement individualized care plans.

Assessment and Monitoring:
• Conduct comprehensive assessments of patients' conditions, monitor treatment plans, and evaluate patient progress to ensure optimal outcomes.

Medication Management:
• Oversee medication administration and ensure compliance with medication protocols and regulatory requirements.

Wound Care Management:
• Supervise and support nursing staff in performing wound care treatments and ensuring proper documentation and follow-up.

Staff Training and Development:
• Provide guidance, support, and mentorship to nursing staff, fostering professional growth and competency in sub-acute care.

Quality Assurance:
• Participate in quality assurance activities, including audits, performance improvement initiatives, and regulatory compliance.

Communication and Collaboration:
• Communicate effectively with patients, families, physicians, and other members of the healthcare team to coordinate care and address concerns.

Documentation and Reporting:
• Maintain accurate and timely documentation of patient assessments, care plans, and interventions per facility policies and procedures.

Qualifications for Unit Manager:
• Current licensure as a Licensed Practical Nurse (LPN/LVN) in the state.
• Minimum of 2 years of clinical nursing experience, with previous experience in sub-acute care preferred.
• Strong leadership abilities with the ability to inspire, motivate, and mentor staff.
• Demonstrated clinical competency in sub-acute care and knowledge of nursing principles and practices.
• Excellent communication and interpersonal skills, with the ability to effectively communicate with patients, families, and healthcare team members.
• Ability to work collaboratively as part of an interdisciplinary team to achieve common goals.
• Attention to detail and accuracy in documentation, patient assessment, and care delivery.
• Ability to adapt to changing priorities and work effectively in a fast-paced environment.
