Purpose And Scope:

The Outpatient Care Coordinator, an entry-level position in the Clinical Advancement Program (CAP), is a professional registered nurse who is accountable for the provision and coordination of clinically competent care to a specific group of patients. They assess, plan, implement, and evaluate patient care and delegate tasks to direct patient care staff. As part of the ESKD... healthcare team, they engage in decision-making, teaching, leadership functions, and quality improvement activities designed to improve patient care outcomes and the overall facility operations.

Principal Duties And Responsibilities:

To adhere to established nursing practice standards, it is imperative to operate within Fresenius Kidney Cares framework of policies and procedures while considering relevant state and federal regulations.
• Completes all required responsibilities under the direction of the Supervisor and while collaborating with the aid of the Clinical Educator, Preceptor, or other Registered Nurses.
• The healthcare provider assigned to a patient undergoing hemodialysis is responsible for continuously and meticulously gathering and assessing data related to the patient pre-, during-, and post-treatment. The data collected is used to adjust and modify the treatment plan as needed, and the relevant medical entities are notified of these changes.
• Analyzes, collaborates with, and documents the primary learning needs of individuals and their families, with the aim to provide comprehensive education service from the start and subsequently.
• Facilitates the delivery of hemodialysis treatment in a secure and efficient manner, working alongside ancillary and direct caregivers to assure patient safety and well-being.
• Gives medications in accordance with the prescribed instruction or approved algorithm(s), and records the correct medical rationale and efficacy.
• Participates in or helps to carry out emergency response procedures.
• Acts as an important member of the healthcare team, participates in staff development and assists in the orientation of fresh members as allocated.
• Confirms that the appropriate procedures for laboratory collection, processing, and shipping are adhered to and schedules any missed or incomplete laboratory collections to be rescheduled.
• Expected outcomes can be identified and nursing assessments and plans of care can be documented and updated through active collaboration with the Interdisciplinary Team.
• Ensures that patients are knowledgeable and well-informed about transplant and treatment modalities.
• The CAP requirements represent a crucial step towards attaining your goals and objectives.
• Takes care of any other responsibilities instructed by the Managerial Staff.

Physical Demands And Working Conditions:

This job involves unique physical demands and work environment characteristics that workers must encounter when performing essential functions. Adjustments will be made as needed to help individuals with disabilities carry out these duties.
• This role requires direct patient care, necessitating heavy lifting, moving patients, and supporting their ambulation. Support may be provided by team members or assistive devices.
• The employee must have the capability to stand for a long time and bend over repeatedly while performing the duties of the job.
• The worker may have to move large pieces of machinery while receiving help, which can weigh up to 200 pounds, and lift chemical and water solutions of 30 pounds or less up to 5 feet in height.
• A temperature-controlled work environment with moderate noise levels is common in healthcare facilities.
• There is a chance of encountering infectious agents or contagions that pose a health risk.
• The work involved in one's day-to-day life typically involves completing paperwork at a desk, operating computer equipment, conversing with patients, medical and hospital personnel.
• It may be necessary to visit training venues or other establishments for the job.
• The essential job duties of this position may entail working in other locations with identical physical demands and working conditions as outlined above.
• Applicants will be asked to complete the Ishihara Color Blindness examination to be considered for employment. If a candidate is unable to pass this test, it does not mean that they won't be hired. Supervision:

By satisfying all the requirements, one can take on the task of managing Patient Care Technicians/LPNs/LVNs/RNs as a Team Leader or designated Nurse in Charge:
• The accomplishment of all FKC education and training expectations is a requirement that all new employees must comply with to demonstrate their commitment to their career with the company.
• Candidates for the RN role must provide evidence of 9 months of prior experience.
• An individual who has worked in chronic and acute hemodialysis for at least three months is eligible to apply for the RN position.

Education and Licensure:
• A person who holds a diploma from a licensed nursing school.
• The up-to-date state accreditation which is required for legal compliance.
• As a healthcare professional, it is important to possess a current certification in BLS CPR.
• Meeting the practice obligations of the state in which the person is working is mandatory.

Experience And Required Skills:
• A position is available for registered nurses who have had less than a year of experience in nephrology nursing in the last two years.
• Nine months of experience as a Registered Nurse are necessary to be preferred for this role.
• Previous experience with long-term and short-term hemodialysis patients is an asset.
