**Job Title: Physiotherapist
Location: Lower Hutt
Hours: Full time - 7 days service - Will be expected to work 1 weekend day per month with a rostered day off in the same fortnight.
Salary: $60 - $73K per annum**

This is a great opportunity to join a friendly interdisciplinary team, that provides rehabilitation and recovery in people’s own homes. Our Early Supported Discharge (ESD) team offers post-discharge rehabilitation (up to 6 weeks) to people post-stroke and injury as well as brief home-based assessment and support to those who would benefit from an early transition home from our older persons and acute services.

The Stroke and orthopaedic rehabilitation streams of ESD are now well established and the team will continue to expand to support this work, along with flow from ED and our inpatient wards. Within this role, you will be completing holistic assessments, and providing brief, intensive rehabilitation. We will be moving towards more... inter-professional practice approaches. So we are looking for therapists who can key work and who are keen to cross skill and make the best use of what will be a growing Assistant workforce.

In order to offer a 7 day per week service, the therapist in this role will be rostered to work one weekend day per month with a rostered day off in the same fortnight and remuneration as per the MECA employment contract for Allied Health staff. About the team: Our ESD team includes Nurses, an Occupational Therapist, and Rehabilitation Assistants and this is one of three Physiotherapy roles within the team.

We work closely with the HCAs of our Community Nursing team and have Consultant medical staff offering advice in our IDT meetings. ESD is part of Te Herenga Tangata, our broader Older Persons, Rehab and Allied Health team so you will have access to collegial support from therapists working across our Community services. You will find professional support from your Physiotherapy colleagues within the team and Professional Leaders for Physiotherapy. • *Skills & experience required for this Physiotherapist opportunity**

- Minimum 3 years of clinical practice NZ Registration Physiotherapist with current APC Member of Physiotherapy New Zealand
- Professional Association (desirable) Postgraduate qualification in Rehabilitation/Neurology (desirable)
- Knowledge and experience of Enable NZ processes and credentialing
- Clinical experience applicable to the role is essential
- Evidence of knowledge of diagnosis, pathophysiology and clinical management of complex elderly, stroke, neurological and orthopaedic conditions is essential .
- Evidence of knowledge and application of both rehabilitation models and functional approaches to assessment and intervention -
- Current full NZ driver's license with the ability to drive a manual and automatic car.
- Proficient in Microsoft Office, Word, Outlook, PowerPoint, internet resources and email.
- High standard of written and spoken English.
