Responsibilities and Duties:

Direct Patient Care

Clinical care and management of common chronic conditions
Assesses plans and delivers quality care to geriatric patients with complex health needs, who require care of both extended medical functions and nursing focused management.
Collaborates with nursing staff, patients, families, physicians and other health team members in planning care for patients with complex health needs
Formulates differential medical diagnoses through performing physical assessments, ordering and interpreting diagnostic tests
Plans and manages patients in collaboration and consultation with other healthcare professionals. Direct patient clinical care includes health history-taking, physical examination, documentation, diagnostic reasoning, ordering and evaluation of diagnostic tests in collaboration with physician.
Initiates treatments and performs advanced procedures according authorized scope of practice, guidelines and/or protocols
Specifically... in identifying complex care needs for older persons with falls, depression, dementia, delirium, and wounds.
Provide consult service and management of patients with common chronic conditions and complex wound care needs in the outpatient and home setting in collaboration and consultation with physician.
Provides anticipatory guidance, education and counselling for patients and their families
Provide self-management education for patients with diabetes, chronic obstructive lung disease and cardiovascular diseases.
Develops and evaluates patient education resources
Serves as a patient/family advocate
Demonstrates problem-solving skills based on theoretical knowledge, clinical expertise and sound clinical reasoning
Assists with development and implementation of nursing care standards/clinical pathways
Evaluates and improves standards of nursing care and patient outcomes
Develops practice initiatives such as extended role tasks where appropriate


Provides education in area of expertise to staff, community and education organizations
Develops, implements and evaluates educational programs based on identified needs
Facilitates a staff development culture. Provide nursing education and update to be current with best practices through nursing journal clubs, nursing rounds, nurse sensitive outcome audits

Leadership in Professional Development

Provides clinical supervision to nursing staff
Facilitates the development of nursing staff to promote their professional growth, including APN/Interns
Facilitates the management and development of human resources
Assist in the planning, recruitment and retention of staffing needs
Participate in staff performance appraisal as People Manager
Facilitate team building efforts in the wards/units.
Involve in clinical and quality related audits, reviews and training
Lead nursing rounds with advanced diploma trained nurses to improve care
Lead case conference with multidisciplinary team

Consultancy/ Resource

Serves as a consultative resource to nursing staff and other health team members.
Contributes to the determination of health and service needs, evaluates and improves on the services and programs for the patient population
Monitors and initiates strategies to improve the quality of care


Appraises and synthesizes outcomes of research critically
Uses evidence-based approach to patient care management and outcomes
Identifies research questions and conducts or participates in clinical research activities
Communicates research findings at professional meetings or by publications
Support appropriate nursing research at advanced practice role
Monitoring of patients? outcome through audit trails and clinical improvement strategies
Collaborate with academic nursing institutions in research
Develops practice through participating in professional organization's initiatives
Promotes change and innovation proactively
Develops strategic direction for practice and service

Job Requirements:
Preferably a Senior Registered Nurse who has completed post basic training, posessing either a Bachelor and Masters of Nursing with certification recognized by Singapore Nursing Board, and has at least 5 years of experience in the area of specialization.
Experience in palliative care would be an advantage.
Clinical experience in nursing management with evidence of effort in continuing professional growth
