Guide clients and their whānau through their end of life journey and support them to achieve their goals and wishes

This Hospice is on the lookout for a Social Worker or someone with similar experience, and a passion for palliative care. You will be supporting patients and their whanau through end-of-life journeys. This includes identifying and addressing psycho-social, emotional, and bereavement needs, collaborating with a multidisciplinary team and external partners, and facilitating meaningful conversations while deeply respecting te ao Māori and Te Whare Tapa Whā principles.
This team is known for its exceptional care, leading with vision and empathy. Work closely with an excellent leadership team, to deliver the right support.

Provide Holistic Support: Identify and address the psycho-social, emotional, and bereavement needs of patients and their whānau.
Collaborate Widely: Work alongside healthcare professionals, government agencies, and community services to coordinate comprehensive care.
Facilitate Meaningful Conversations: Comfortably lead hui and navigate challenging discussions with empathy and respect.

What you need to have:
Relevant Experience: Background in social work, health navigation, or community-based health/social services.
Professional Skills: Strong facilitation skills and experience in leading sensitive and challenging conversations.
Cultural Competency: Profound understanding of te ao Māori and Te Whare Tapa Whā.
