Overview: The Case Manager II provides case management and supportive services to clients who are exiting street-level homelessness as residents of scattered site small-group emergency bridge housing units. Through site in-reach, Case Managers (CMs) seek to connect guests to sustainable housing. CMs meet guests where they are and support them to achieve their goals. City Net’s services are... housing-focused; CMs work with guests on developing long-term housing plans. CMs also connect guests to appropriate supportive services, including medical treatment, mental health treatment, counseling, and permanent housing.

CMs offer care and service coordination in partnership with other nonprofits, County and City entities, law enforcement, behavioral health services, and medical providers to ensure clients receive the help they need. These services are provided in collaboration with our partners with the intention of assisting clients with obtaining and maintaining permanent housing as quickly as possible. City Net’s programs follow Housing First and Harm Reduction service models.

Main Work Location: Reports daily to the Sacramento Main office to be dispatched to fieldwork throughout Sacramento.

Work Environment & Expected Hours: This is a regular, full-time, hourly position. This position works five days per week, 8 hours each shift for a total of 40 hours per workweek with optional overtime opportunities available.

Schedule: Days and Times to be determined in collaboration with the hiring supervisor.

Please note: while this job posting is for an opening we currently have in the City mentioned above, we are responsive to the needs of our clients and funders, and therefore the person in this position may be transferred to neighboring locations as needed.

Requirements
• Minimum 1 year of professional social services case management experience (ideally with City Net’s target populations--people affected by homelessness, substance use disorders, chronic health and mental health concerns, domestic and intimate partner violence, and/or human trafficking)
• Upbeat passion for ending homelessness
• Excellent professional boundaries
• Strong interpersonal, analytical, and time management skills
• Effective and professional networking and relationship management skills
• Understanding of the importance of quality and timely data collection and reporting
• Professional verbal and written communication skills (English)
• Ability to work independently and as part of a team
• Proficiency in PCs and Microsoft Office Suite
• Ability to accomplish goals and produce valuable results with minimal supervision
• Ability to handle multiple cases simultaneously while maintaining creativity and strategic thinking and continuing to meet or exceed goals
• Ability to work well with people from diverse backgrounds with varying degrees of experience
• May be required to drive a company vehicle to transport clients. For insurance purposes, drivers must be at least 25 years of age, have a valid CA Drivers License, and must pass a Motor Vehicle Report
• Must be authorized to work in the United States for any employer

Nice to Have
• Experience and homelessness services professional contacts in Sacramento and surrounding areas
• Experience gathering and maintaining confidential client records, including intake paperwork, case notes, and client management system data entry
• Bachelor’s degree or higher in Social Work, Human Services, Sociology, Psychology, or a related field
