StarCare Specialty Health System seeks to hire a qualified and compassionate Veteran Case Manager to join Homeless Veteran Reintegration Program (HVRP) Team in Lubbock, Texas.

HVRP Veteran Case Manager Responsibilities...
• Works under the direct supervision of the Homeless Veteran Reintegration Program (HVRP) supervisor with moderate latitude for the use of initiative and independent judgment.
• Conducts outreach to the community as part of the HVRP plan.
• Develops substantive networks among supportive service and employers that advance the interests of unemployed low income veteran families experiencing homelessness.
• Screens and enrolls eligible veteran families and provide community referrals for ineligible applicants through a coordinated entry process.
• Provides case management and administers interventions and services approved by the HVRP supervisor, including job trainings and workshops.
• Completes timely and accurate reports and documentation necessary for program compliance including appropriate expense management.
• Advocates for homeless veteran families and the HVRP Program with a vision towards ending veteran homelessness in the community.
• Perform other duties as assigned.

Schedule: Normal work hours are 8am – 5pm, Monday - Friday with flexibility to meet the needs of the program.

• Bachelor’s degree from an accredited four (4) year college or university with major course work in Human Services or a related field, preferred.
• Some college and/or high school diploma and two (2) years of experience in veteran services will be considered.
• Must be a veteran or member of the U.S. Armed Forces (any branch), have experience as a veteran caregiver, or have direct work experience with state or federal military and veteran programs.

Additional Requirements:
• Must have skill in operating standard office machines and personal computer with intermediate or greater experience with Microsoft Office (Excel, Word, email, etc.).
• Some regional travel is required with mileage reimbursement at state approved rate.
• Selected candidate must have a reliable transportation, a valid Texas driver’s license, and be insurable under agency insurance.
• Must pass a pre-hire criminal background check and drug screen.
