Hennepin County Health Care for the Homeless (HCH) Program is seeking two Senior Psychiatric Social Workers to join our clinic teams to provide short-term intensive case management to adults with acute mental health symptoms and diagnoses, substance use disorder diagnosis and complex health needs. We are looking for individuals who are skilled and experienced in working with individuals... experiencing homelessness in sheltered and unsheltered settings to provide clinical services and team-based care. In this role, you will work closely with clinic staff to provide wrap around services and mental health services to patients of the clinic. This work supports the Health and Human Services mission of improving the health of our communities and the goal of assuring that all people's basic needs are met.

Location and hours:These positions are hybrid and will be performed both on-site at various Hennepin County locations, and remote as job duties require. Work hours will typically be Monday through Friday, 8 a.m. - 4:30 p.m. with some flexibility. Additionally, these positions may require working one evening a week. While these positions are designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In these positions, you will:
• Work collaboratively with the HCH Medical Clinic Team in following objectives such as building relationships and trust, promoting harm reduction while identifying health needs, and assessing readiness to engage in healthcare, as related to the public health priorities of the opioid crisis and drug related infectious disease work.
• Collaborate with other homeless community provider agencies and Hennepin County departments.
• Provide clinical services such as brief therapy, diagnostic assessment and crisis assessment.
• Participate in Quality Assurance and Quality Improvement projects with HCH clinic teams.
• Document data, plans, clinical actions, client progress, response to care, and other relevant patient information in Epic and in compliance with statue and rules of confidentiality.
• Participate with staff on broader Hennepin County projects.

Need to have:
• Master's degree or higher in social work or closely related field and two or more years of post-master's degree experience in a psychiatric or mental health setting.
• Eligibility for or licensure as an Independent Clinical Social Worker by the Minnesota Board of Social Work.
• Must qualify as a Mental Health Professional as defined by the Minnesota Mental Health Act.
• A valid driver's license, the ability to obtain a Hennepin County driver permit, and access to reliable transportation to travel to various locations within Hennepin County.
