Salem Health is seeking an experienced Care Manager to join our team. Our ideal candidate will have a passion for helping others and a dedication to providing the highest quality care. With a keen eye for detail and an ability to stay organized, the Care Manager will ensure patient care is coordinated and delivered efficiently. The successful candidate should have a degree in Nursing or a related... field, and a minimum of two years of experience in a clinical setting. A valid nursing license in Oregon or the ability to obtain one is also required. Experience in a managed care setting is preferred. We are looking for an individual who is driven, organized, and has excellent communication and interpersonal skills. The Care Manager should be an independent worker who is able to prioritize tasks and make decisions in a timely manner. Being a team player with a positive attitude is a must. Responsibilities:
Coordinate and deliver patient care in an efficient manner
Stay organized and oversee care plans to ensure they are being followed
Ensure all patient medical records are up to date and accurate
Maintain a valid nursing license in Oregon
Communicate with patients, families, and other healthcare professionals
Prioritize tasks and make decisions in a timely manner
Monitor patient progress and ensure that the highest quality of care is provided
Remain current on changes in the healthcare industry
Participate in team meetings and work collaboratively with other departments
Create and implement patient-specific care plans
Utilize resources and technology to improve patient care
Foster a positive and supportive environment
