What you will be doing
The project will establish a sustainable approach to better identifying and supporting older people, including those with dementia and their carers in the community who may frequently attend ED, in hours or after hours, to ensure transfer to hospital and hospitalisation is minimised and that appropriate care planning and supports are available in the community.
The project will develop a service model to support Improving Outcomes in Aged Care in the Community which will enhance and build upon current Integrated Care Models.
